Description

To provide a mechanism for reviewing professional performance of a physician or credentialed medical staff member reviewed by another practitioner of “like specialty” to determine the appropriateness of clinical decision-making and overall quality of care.
Peer Review process is mandatory and you will need to fill out the following forms that are included here:

Physician Peer Review Form
Anesthesia Peer Review Form
Chart Auditing Form
Facility Nursing Peer Review Form
Peer Review Form for Patient Transfer
Completed Peer Review Process Form to place in Credentialing File
